Documentation Changelog

Append-only, most recent first. One entry per pass that materially changes what a doc claims or how the tree is organized — not a mirror of every commit, and not a record of what the product did (that’s git history and the product’s own release notes). A pass that only fixes a typo or a cross-reference doesn’t need an entry; a pass that changes what a doc says is Built/Planned, adds or removes a doc, or restructures the tree does.

When adding an entry: date it, name the docs touched, and say why — matching the status / lastReviewed discipline already used inside each doc’s frontmatter, one level up at the tree.

This changelog starts from the tree’s current shape — it does not carry forward the history of how documentation/ got here (rewrites, archives, renames). Only the current, implementation-matching state of each doc matters going forward; git history is where prior states live if anyone needs them.
